The QA team and myself have not been able to reproduce this.
We've tried holding down ctrl-shift while watching others kill themselves, repeatedly hitting shift while holding down ctrl so the object handles flicker while watching others kill themselves, tried dragging health bars off these object handles while watching others kill themselves, and having others kill us while doing the same variations mentioned above.
There might be some other factor that's unaccounted for.
Is this location specific? Are you running the client under a different language?

I have had this happen as well.
1. Not easily reproducable; does not seem to happen every start-up. It's like something is going wrong in program initialization when it does occur, as it will happen every time when the chain of events creates the conditions for it, or not at all, if whatever bug is setting the stage for it does not occur.
2. Classic Client, Standard settings for language, localizations, etc.
3. Using UOAssist.
4. With me, it was when a PET died, as I was trying to use CTRL-SHIFT to grab its bar, after losing its bar from other reasons (like having it die, I rez it, then it charges, unhealed, after the nearest target, before I grab its bar again. Usually, getting its bar back up before rezzzing prevents this, but sometimes one forgets, due to the haste of combat).
5. It appears be be from the action of dragging during the death process.
6. This doesn't occur, interestingly enough, if a Goblin* dies as you're dragging its bar.
* You might want to look into this - Goblin's bars remain up from CTRL-shift after death significantly longer than any other creature's. Don't know why, but it's just something I noticed while hunting gears.
Unrelated, but important - Billy, look for my post a few pages down about getting "Telestormed" out of the Underworld after an apparent glitch on the Underworld's subserver on Lake Austin about a week ago.
That section of the underworld with the Gray Goblins, Spiders and the Navrey entrance appears to be unstable and prone to freezes on Lake Austin. I went there again with a friend/fellow alliance guildleader, to hunt. She's in TX, I'm in KY, with completely different paths to the LA server in CA, and every time one of us experienced a freeze, sometimes as long as 3 seconds, both of us experienced it.
