I have had it happen on creating soulforges and you do lose ingredients -- not the relic fragment though. I didn't use that button. I'm always making a certain number, and the number changes so I use that every time.I have noticed this too. It seems to be the "Make Last" button thats messed up for me though. I i click on the make item button it seems to work just fine, but whenever I use the "Make Last" option is when I start failing.

You DO loose relic fragments at times if failing even at 100%!
Well actually, what I said was true. I was only saying that I had not lost a relic fragment in the two fails.Not true!

The only time I ever saw that happen was when you try to cast twice because of recall lag.My legendary mage fails recalls all the time... Welcome to UO.

I know, in a mathematical view, this is not very convincing due to the few attempts made; nevertheless, with these numbers above, I suspect the success chance simply remains @ 50% ignoring the bonus of any equipped talisman.
Just because your skill is at 100% does not mean you will succeed 100% of the time. Look at the menu and see what your chances of exceptional are. Odd, given that they were discussing this pre-release, and had mentioned they would look into ways to not make the imbuing failure process so painful. Do you think this is possible with other spells? I'm fairly certain I fizzled a pain spike on a 100 necro yesterday...The only time I ever saw that happen was when you try to cast twice because of recall lag.
