Ok, went to Terathon Keep..killed hundreds of Terathon avengers. No arms. What am I doing wrong?

kill any terathon and you'll get the arms - looks like a dead plant.
I have done the quest and can now make the stew on Europa.

Maybe he has another quest on so he cannot get the arms?

All Avengers are supposed to have 1 as loot. Since nobody (*points finger*-->Cal) thought to link the 2 complete sets of Ophids in the game & make them know who their friends are, Most people probably won't make it in there to find out and are not gonna try, thus the appearance of secrecy.
What that did was ruin the quest for people.
What's a secret is how they keep UO aloat pushing out stuff with this quality.
This, In addition to the almost impossible task of collecting a full set of virtue armor, as a Quest item, this whole storyline is really turning to crap

I suggest an appeal on your shard board for a rune to the keep. Most shards will have people who marked runes on the path outside.
I just checked this, not only did I get arms on the character that has finished the quest, I also got them on the char who has no loyalty to the ophids and hasn't taken the quest. Here's what you're looking for, the dead plant graphic:
