Quiver of Infinity Bug

So this happened three times to me warrant posting it here.
Had a friend convert a quiver of infinity for me 3 separate times at the Ter Mul soulforge. After converting it and trading it to me, it "poofed" there was one instance when it poofed in my friends bagpack.

Is this a known bug? Anyone had this happen? And I suppose reimbursement is out of the question?

I can say it happened to a friend of mine, I converted the quiver and gave it to him. Its not in his pack and he killed himself and I killed myself and its not on either body.
