Use the Enchanced Client: User Settings, Input tab, Scroll Wheel Up: set to Cursor Target Self.

UOAssist is the only (legal) way I know of to make the mouse wheel work for the classic client.
On a side note - its always made more sense to me to roll the mouse wheel down - or towards myself - to target myself. I use mouse wheel up for last target.

How I do it is i set the window manager mapping to map the mouse wheel to a key on the keyboard, then set that key on the keyboard to the action in UO (so the mouse acts like a keyboard key which is all you can map in the game). Just pick a key you're unlikely to hit otherwise ingame, i generally use [ and ] for mouse wheel actions.
use x-mouse buttons and you can set any macro to the mouse wheel.
Note: this program is like the one given with the razer mouse so it has nothing to do with the approved softwares.
