Is it true that you cannot use a shuriken if you are closer than three (3) tiles?

I believe it is three.
Hit chance is based upon Ninja skill. I believe it is compared to target's weapon/defense "skill". Not sure how HCI and DCI affect this.
That said, I've found some players can't be hit at all(wasted 3-4 belts full of stars on them without a single hit)

Anyone know how to load the Shuriken's into the belt?

Context menu on the belt. Right click or shift+right click. Pick load.

Welcome to UO!
Shuriken don't stack normally, but you can stick up to ten in a belt at once then unload it (getting them all back as a single item).
