I notice in tram only, the dungeon Despise by the Earth Elemtals...There seems to be a new higher swamp now that you cant get to.

There is also an area in Despise near the titans that appears to be inaccessible and yet the titans and cyclops will spawn inside making them impossible to get too.
additional area by the acid elemental too, been there quite a long time now, but no word on its function.

This is where the dev's go, in game, to take their coffee breaks. Or perhaps something a bit stronger at the end of their work week 
is there a table for there donuts?
oh yes and they did not stop at a table, just in case they have to work late they got there own huts too.

No, I doubt you'll get into a dungeon from another dungeon. It will probably be entered the same way as it was in the single player Ultima games.
