I like the idea,but there should be more to it than just a title. Perhaps having a title higher than lord/lady should grant you access to some new "something" offered in the game.

Long ago, high Karma earned you an NPC vendor discount... High Fame still causes some NPCs to bow to you. Since both are so ridiculously easy to raise, we shouldn't expect them to do anything. Think of Fame/Karma titles as what they are: vanity.

IMHO titles should be put "for sale" and we should consume karma/fame to buy them.
Only this way will allow to see more uncommon titles around, because karma/fame may vary too quickly and the title will reset everytime to default (especially now that we burn sacrifice at every use)
