I'm going to ask you a very stupid question, and you are going to forgive me, and answer it anyway.

Have you restarted the client? Have you restarted the computer?

Why would these work or be relevant at all? The same reason sometimes items vanish for no reason, only to reappear. And the same reason that sometimes people's profiles vanish. Or their desktop icons disappear.

Primarily, the timer on polymorph should be removed again.
In addition, polymorph needs more forms.

Remember that forms etc can not be maintained if you do not have the skill level for real? Casting an animal form while 'jewelled up' and then taking the jewels off will have that effect now.

I've said since the intro to Samurai Empire that the mage should get certain benefits from the creatures they polymorph into, ala ninja dog form for hit point regen... or if you poly into a demon you get additional spell damage increase bonus, or something like that.....etc, with the rest of the poly spells.
