reformating hard drive

Im sorry to bother you all. But i need help. Ive reformated a hard drive before but it has been a very long time. Can anyone here please give me instructions on how to reformate my windows 98 hard drive. I have a floppy of windows 98. I just get confused after i type in FDISK and the first menu comes up with 4 choices. I want to clean my hard drive and reinstall every thing. I have every thing i need to reinstall i just need a little guidence. Thank you for any good help.
