1. Pets cannot be ordered to attack other player characters, but will defend
them if told to guard. (same way it currently is with pets vs robotic critters).
2. Adjust Stealthing so that it functions more realistically.
a. Must toggle into and out of Stealth Mode.
b. Movement rate is cut in half, including animal form.
3. To cast spell you must...
a. select spell.
b. select target.
spell is THEN cast and immediately directed at target. No running around
with spell pre-cast, seeking a target.
4. All Field and Area spells effect EVERYONE, even caster. Only exception
being spells that when cast are centered on caster, in which case only the
caster is exempt from effects. (Whither/Earthquake)
5. Bring back old armor system, where metal armor gives better protection,
but comes with dex penalty.
I know you only asked for 1, but the problems with PvP are too encompassing
for a single change to do much good.

If I had to pick 1, pets.
Stealthing is a problem because detect is too weak (well that and smoke bombs but that's another issue). Everytime I use a skill check in stealth, it should make a roll against any detecters in the area. This isn't a passive reveal, but a failed active stealth check. So essentially, if I use the stealth skill (and I do every 16 steps), it should check against anyone with detect and based on their detect, my stealth could fail. And yes, I am a stealther.

I don't think Stealthing is a problem because detect is too weak, I think it's a problem because Stealth is too strong. But neither should we turn the clock back to the way Stealth was way back when. Back then it was almost worthless. Have to reach a middle ground would be best I think. And I to am a Stealther.
