If you are running in full screen the option does nothing, if you are windowed it would remove the "frame" from the Windows form. basically the title bar and the rest of the frame that goes around it.
on some computers, mine for example, it makes a phenomenal difference to performance. playing windowed with it on I can barely move, the cursor goes across the screen in erratic jumps making it virtually impossible to get it lined up on anything to select it. Turned off I can play windowed as easily and lag free as full screen.
