Returning player... how to log back into account management to resubscribe?

When I left the game years ago, I just had a UO username and password. Now when I go to the UO account management link, it says something about an EA account... ?

How can I get logged into my uo account to resubscribe? Or have I now lost everything I had on that account and have to start completely from scratch? please give some suggestions.
