I found this ORIGIN - Ultima Online - Main for the recruit a friend, is it still active? I have a real life friend who wants to play UO and he asked me to look for one of these deals to help us both out.

Recruit a friend to uo, get him to decide what template to play first, if he plays a dexxer get him to download EC and download pincos UI, if he wants to be a mage get him to download CC and buy and download UOassist, then get him to download UOCartographer, follow the instructions on how to get it to work. If he plays CC get him to change the "Keynames" file so he can actually record macros on the numberpad. On either client get him to change the default settings for pretty much everything to make the game playable. Then you're good to go. 
Ok, I looked around abit and after the merdger that EA did to link accounts, I don't see anywhere for a recruit a friend to UO. All I have found is this.

Here's how it works:
- When you purchase Warhammer Online: Age of Reckoning, you can invite your friends and family to try the game. You can send one (1) Recruit-a-Friend invite each day. If an account is created that day, you can send another.
- Send a referral to a friend or family member via a convenient email. This will allow your recruit to create a free trial account. You may recall unused referrals at any time, allowing you to re-send a referral if the one you recalled was sent that day.
- When your recruit becomes a paying subscriber to WAR, you will receive a credit for 30 days of free game time. Recruit one (1) paying subscribers to WAR and you will also be awarded an exclusive in-game pet to accompany you on your journeys. Recruit two (2) paying subscribers and you will be awarded a special in-game item. Recruit three (3) paying subscribers and receive an exclusive mount for your characters."

If you have a non american keyboard, and you want to record macros on the numberpad, you must edit a file in uo called 'keynames'
open up the text file and the defualt name of the numberpad keys are:
num1
num2
num3 etc
you need to change this to
NUM1
NUM2
NUM3
This is dependent on the language of your keyboard, check your character macros to see what they call the numberpad keys to get a true match.
otherwise it doesn't save your macros on the numberpad, you'll find after each patch now you'll have to repeat this, perhaps backup the edited keynames file somewhere, along with your character macros and replace it after each patch

Account merger is what I was thinking, they suspended the program just prior i think. I do not remember seeing a re-activation notice.
Put in a trouble ticket to CS by email - trackable at least, probably quicker and more accurate than a GM response.
UltimateGameCard still has this listed as an item for sale (not sure if your original link info was a 'free' offer?).
