A bit of Pinco's interpretation note here since it looks like most of the other listings are not done with Pincos:
Greater artifact (4 yellow stars in the upper left).
And note the 5 Poison Resist which is listed on the resists instead of the normal attributes listing.

Thanks for the response, I'm fairly new back to the game, trying to get a gauge on what's worth a lot and what's not even worth saving still.
