Old items messed up in a recent patch?

Two things are giving me the "Token out of range" error now! One has the dead plant graphic but I'm pretty sure it was a guildstone deed back in April; the other has the crystal ball graphic and might've just been a pet ball.

I saw some oddball tiles in Deceit on a recent visit - in fact one of them blocked me in a hall.
