Treasure Map Chests, SOS Chests, and other loot sources occasionally spawn "base" (no properties) leather items, even though the chests and creatures are not supposed to have such items in them.
These are specifically male and female leather armor items and cloth hats; presumably, this may also occur on some ML creatures with leaf armor, but as ML loot does not appear in the chests, it is near impossible to test.
This is an OLD, OLD bug - specifically, it can be traced back to where the loot system AND Runic crafting system, circa Age of Shadows, would attempt to place "Mage Armor" on loot items that already allowed Meditation.
This was fixed several years ago for runic-crafted items, but is still present for loot (again, most obviously, in the treasure chests of the Map and SOS systems). Typically, you'll run into one of these pieces about once every 2 or 3 high-end T-map chests, though I have had chests with two such items (and once, three items). It is likely that items that seem to be weaker than they should in the revamped dungeon systems might also be falling prey to this.
This should be an easy fix, as the fixed code was already introduced into the Runic crafting system. It might be advisable to also double-check the Reforging system to insure this bug does not exist there (since it is more closely related to loot systems than to the existing item creation systems).
