I'm 100 in meditation yet when I go to activate it, probably 30% of the time I'll get the "You cannot focus your concentration" message. From my understanding 100% in something means you have 0% chance of failure?
EDIT: Ok I think I just figured it out. I started using my Crystal ball of Knowledge and I noticed that if I activate meditate right after I killed something it said "very challenging" but if I waited a few seconds it would say "optimal" and if I waited even longer it would say "very easy". So if waiting a few seconds after combat before activating meditation was common knowledge, I feel like a dunce heh.
