I belive its from the lich spawn. Maybe wrong though!

It is from the new lich spawn. During the spawn, random items will come "Unlocked" and are able to picked up. That is what the lever in the picture is as well.

seems to me the items dont "unlock" the replica will spawn next to it, thats how the items ive gotten have been, ive gotten a half of grave, different skull pile, and brambles and they were always next to one i couldnt pick up.
