in need of the template for a Wammy

Its basically the same only you can have lower Necro and need SS.

Yeah you'll want to be able to stay in Wraith Form and cast Curse Weapon 100%, which I think is at 60 but you should check the minimum skill (in the book) plus 40 gives the 100% point.

For SS, I would have 100 minimum.

You might be able to cram Chiv on there, but your main source of healing comes from Curse Weapon which gives 50% Life Leech. The constant casting of Curse Weapon usually means your Karma won't be high and the duration of Chiv spells will be affected.

I'd drop Necro by 20 and bump tactics and antomy both up to GM. You get both a damage bonus and an additional 2 seconds to Evasion. You'll also want to have Protection cast on yourself so you can't get interrupted casting Curse Wep.

Drop healing, add chiv IMO. You're leeching 50% of all damage you do. That 50% damage increase from EoO, and Consecrate Weapon are much more useful than healing with wraith form and curse weapon.

... well since you wont be casting any real necro spells anyway, just use a protection scroll so your curse weapon doesn't get interrupted.

Oh and, if you don't mind constructive criticism, you're kinda shooting yourself in the foot here by having less than 100 necro.
With full necro you can switch between Vampire and Wraith form as the situation requires.

70% damage leeched and 20% damage reduction can be useful... in some rare, obscure instances i suppose.

...granted you're probably using this char for completely different purposes than I do with mine so you're gonna typically get a lot less use out of Necro in PvM and with no fc/fcr.

Depends on what you're killing most of the time. If you're doing spawns, karma will stay high, if you're killing bosses, you're karma's wrecked (until you do a spawn, then it's high again).
Without FC/FCR Close Wounds is too slow, as is remove curse. With FC/FCR the only real PvM negative of having low karma is Remove Curse wont work. Cleanse by fire will work, but you'll fail a few times, but curing poison with it isnt that useful anyway since only High level poison matters, and they're only inflicted by monsters that inflict it on hits instead of spells, and then they'll just infect you again 5 seconds later. Not to forget, Curse weapon typically just laughs off the extra poison damage.

Also, at 70 chiv you still heal 25 damage with completely negative karma.

Yeah. You can't cast wraith form 100%, but when you do cast it, you stay in that form til death or you have that new mysticism spell used on you. Curse Weapon is 100% at 40 though.

Hows the plate been working out too dude? I'm having similair thoughts myself... though admittedly, with less points to play with...
