you would just simply start paying monthly for the account or purchase up to 6 months in advance from the UO.com site. Log into your account from the Account Management tab, and go under subscription. You may also want to upgrade to the current SA *stygian abyss* Expansion, which you would choose the Upgrade code..... if I'm wrong, I am sure someone will correct me hehe 
You are correct, just go into the Account Menu on UO.com website & put in your CC info. It will be on the left side under the heading of Account Mgmnt tab, click the tab & then click on the tab New/Edit Acct, that will bring you to another screen & in that you will have to enter your UO user name & PW to get into it to do your changes. It takes just a very few min's to do it all. GL

Think you have to buy upgrade code for SA though.
you need to subscribe for billing as others have stated, then you will need to buy the Stygian Abyss upgrade code, but I do not think you can upgrade your account until it is 30 days old (one billing cycle if you pay monthly)
So I have my trial account.
I used a gametime code on it.
I also have my credit card information on the account.
and it still won't let me eat a powerscroll...
I see that Mondain Legacy is active on trial accounts so I don't need to purchase Stygian Abyss.
Am I missing something here?

It takes a day or two until your account status is updated.
I would not waste my time with trial accounts. Im sure you can find a 4 year account very cheap, even as cheap as upgrade code or close to it, and have the extra 20 skill points added to cap, and the added stats. Also be able to ride mounts of the ethereal nature
