How to use poisoning?

Should I apply poison on my weapon? If yes than how often should I do it?

Is it useful in pvm?

Poison isnt really useful in PvM imo. When you apply it to your weapon it gives you a count of how many charges you have. Once they are gone, re-apply. I used to run a nox/deathstrike ninja back in the hayday of pvp and it was beast.
