I'm trying to figure out how in the world to access the little island in the underworld that has the bloody spoon and mystics guard stealable but no such luck. Does anyone know how to get over there?

Gargoyles can fly over there by activating their "flight" racial ability.
Humans and Elves will need to use a relocation spell such as teleport (magery) or shadow jump (ninjitsu).
If you are human, you can teleport using a teleport scroll. Buy them at a scribe/mage or get somebody to make you some.
