What happened to Fire Isle?
I see it now has the entrance to the Underworld. What is the story behind it?

yeah - check some of the back-stories about a month or so before to shortly after SA launch. Fire Isle got a bit of a facelift.

why cant i enter the underworld from fire island? do i need to do a quest first?

First, you account needs to be SA enabled.
Then you need a quest to enter the abyss itself. There are two possibilities. From Fire Isle there is this quest, and from Ter Mur is this.
The Ter Mur quest is a lot easier, and you can now access ter Mur directly by public moongate.
