Failure Rate for Animal Form?

Does anyone know what the failure rate for each animal form is? Or know a link i can look at?

The FAQ covers the min skill needed to use each form and the skill level needed for 100% success for each form.

Basically you have a 25% chance of success when you are at the min skill level. Your chance to succeed increases by 2% for each one point your skill increases above the min skill. Once your skill level is 37.5% above the min skill level you have 100% success for the animal form.
