Does anyone have the exact formula for how evasion is calculated?

Evasion hasn't worked since Bleak gave it a pancaked-up double-nerfing in November 2015.

Evasion does work in pvp and pvm. Im pretty sure you have a higher chance to evade the higher your parry is. I would love to see an exact calculator as well.
