Will 5 mod items be giving off the best materials/whatever they are called? Even if it's just a weapon with like....45hld 47hld air elemental slayer 50 stam leech 50 life leech?

There's a thread in the sticky threads that answers this.
When you unravel, the system determines what resource to give you based on the total intensity of all mods on the item.
An item with the mods you described would probalby get the highest quality resource, because it is probably higher than 400% intensity (I'm assuming 45 HLD and 47 HLD are near 100% intensity, which they may not be if it's a slow weapon)
