I was trying my darndest to apply the code to my account last night, and every single time it would fail when it got to the third screen (it puts up your contact info and asks you to say ok... why?? But ok....)
Anyway, I gave it a few hours then tried again and it still failed.
Then I had the idea of trying IE instead of firefox. Oddly, the whole process worked perfectly at that point.
Does the EA account website have some issue with Firefox?
You might have someone run through the process with firefox, chrome, IE 7, 8 and 9 and just make sure your site doesn't have any odd issues with individual browsers again.
