On a Necromage for example:
120 Eval
120 Mage
120 Necro
120 SS
120 Resist
120 Med

How do they defend against being hit on every single attack? Do they have to carry a spell channeling mage weapon? Or?

In very basic terms - stay out of reach. Spellcasters aren't meant to go toe-to-toe with opponents.

Jewels and/or shield with dci may help a little too.
