uo patch, another instance is allready running

Morning,
Last night I booted up, there was a patch and then nothing. Now when I try to start UO I get the blue box stating that it is allready running. And there is nothing running. Is this a known issue or would I be better off doing a complete uninstall/install this weekend? Thanks.
