V-sync syncs the game's FPS to your monitor's refresh rate. If you use an old CRT monitor, leave this disabled - if you have a new LCD monitor, enabling this will give you less "tearing" when the screen moves.

Hardware death effect is just a "cooler" looking effect when you die. You'll only notice it when you die. I think it adds rippling shadows to the edge of the screen or something like that.
